How to Apply

Submit the completed Adoption Reimbursement Application(DOC - 35 KB) to the Personnel Cabinet’s Office for Employee Relations, Division of Employee Benefits, State Office Building, 3rd Floor, 501 High Street, Frankfort, Kentucky 40601 with the following:

1)     Finalization of the adoption;

2)     Certification by the Secretary of the Cabinet for Health and Family Services that the adopted child is a special needs child, if reimbursement for special needs adoption is sought, and

3)     A copy of an Affidavit of Expenses(DOC - 28KB) related to the adoption files and approved by the court at the time of finalization of the adoption.

If both of the adoptive parents are executive branch state employees, the applications for financial assistance shall be submitted jointly.

Submit the following to your appointing authority:

1)    Completed Adoption Reimbursement Application, and

2)    Adoption Reimbursement Notification Letter(DOC - 23 KB)

Employees who wish to adopt a child have access to the same benefits that are offered to birth parents. Adoptive parents are eligible for Family Medical Leave.

The Family Medical Leave Act (FMLA) entitles you to:

  • Twelve (12) weeks of unpaid, job-protected leave.
  • Having the state's share of your health and life insurance paid.
