Requests for Pathlore Crystal Reports
In order to provide better service to the agencies requesting reports out of Pathlore, GSC requests that you use the following form to submit your report requests. Please allow a minimum of 5 business days for GSC to fulfill your request.
Completed request forms may be emailed or faxed to Jamille Smith at (502) 564-8056.
The form is available in either an Excel or PDF format.
