Electronic Version of the Evaluation Form

The following directions are for using the Updated Electronic Evaluation form which completes calculations.

COMPLETING THE PERFORMANCE PLAN SECTION OF THE EVALUATION FORM:
  • TAB to go from one field to another.
  • The Continuation Sheet is located on page 6 under Special Tools. (Use only when due to lack of space on the evaluation form.)
  • Once the Performance Plan portion of the form is completed, print this portion of the form. (Remember to print the continuation sheet if used.) Employee, evaluator and next-line supervisor sign the Performance Plan in Red ink.
COMPLETING THE FIRST AND SECOND INTERIM REVIEW SECTION OF THE EVALUATION FORM:
  • After the completion of the Interim Review Meeting, evaluator must print the Interim Meeting Documentation to obtain the dates and signatures of the evaluator and the employee in Red ink.
  • Second Interim Meeting Print Tool on page 7 under Special Tools: This tool is a "read-only" reproduction of the information entered in the Second Interim Review Meeting area of Section II. As you enter the Second Interim Review documentation, the information "rolls over" to this print tool page. If you choose to use this tool, you may place the original document from the First Interim Review Meeting into your printer and print just the Second Interim Review documentation on the original. We strongly recommend that you first practice on printed blank forms to determine the proper placement of the original into the printer. Employee and supervisor sign the Interim Review in Red ink.
COMPLETING THE ANNUAL EVALUATION CALCULATIONS ON THE EVALUATION FORM:

RATINGS CALCULATOR

  • The Ratings Calculator is a reproduction of Section 1, Performance Plan (the first two pages of the evaluation form) and the continuation page. Information entered during the planning process on the first two pages of the evaluation form and the continuation page will roll to this ratings calculator.
  • The Ratings Calculator is located on pages 8 - 11 under Special Tools. To use the ratings calculator: Tab over to the determined rating. Either use the space bar or click on the determined rating to indicate rating chosen.
  • Requires that all the calculations of page 2 be transferred to the original evaluation form which contains the employee's and supervisor's signatures/dates as the original evaluation was developed with the intent that all ratings and math calculations were to be placed in Section I of the evaluation form.
  • The rating calculator for page 1 of the Performance Plan and the continuation page can be printed and submitted as the original as there are no original signatures required on this page.

NOTE: THE RATINGS CALCULATOR IS NOT TO BE SUBMITTED WITH THE HARD COPY OF THE EVALUATION FORM.

TO UTILIZE THE TOTAL EFFICIENCY OF THIS ELECTRONIC RATINGS TOOL:
  • Use pages 8 - 11 under Special Tools to do an electronic rating so total points, the overall total points and the overall performance rating will automatically be calculated as you mark the appropriate checkbox.
  • Submit the printed version of the first page of the ratings calculator to replace page one of the Performance Plan. (This is possible as there are no original signatures on the first page.)
  • Page 10 of the evaluation ratings calculator is a "read only" reproduction of the ratings information entered into page two if the Performance Plan. This will allow you to print the electronic ratings on page two of the Performance Plan which reflects the original signatures in Red ink. If you choose to use this tool, you may place the original document from the Performance Plan into the printer and print just the ratings and math calculations on the original. We strongly recommend you first practice the printing process in blank forms to determine the proper placement of the original form in the printer paper tray.
SPECIAL TOOLS SECTION
  • Pages 5 - 11 are special enhanced tools (forms) which aid in completing the Employee Performance Evaluation. For more detailed information regarding the Special Tools, please refer to page 5 of the E-form version of the evaluation form.

PLEASE NOTE: After completing the annual evaluation, only submit the completed four (4) pages of the evaluation form containing the original signature, ratings and the continuation sheet to your agency's central personnel office.

UPDATED 32-BIT CLIENT DOWNLOAD AND INSTALLATION INSTRUCTIONS

This E-Form version of the Annual Employee Performance Evaluation requires the download and installation of a free 32-bit client software. If you have used the older E-Form version of the Employee Evaluation provided through the Kentucky Forms Management Program, you will still need to download and install this more recent version of the 32-bit client.
