Q: Where can I get a copy of our agency's organizational chart?

A: High level organizational charts can be found on our cabinet's website at: http://personnel.ky.gov/persadmin/info/orgcharts.htm. If you do not see the chart you need, special requests can be sent to Dawn Moreland at Dawn.Moreland@ky.gov

Q: How can I have a name changed in COS?

A: Names changes should be submitted on the COS User Access Request Form with the "Type of Request" box marked as [Modify].
