A: It doesn't cost anything unless you have received a copy before. If you have received a copy in the past twelve (12) months, then it will cost $0.10 per sheet of paper printed, If there is something new in your file that you didn't receive a copy of before (past twelve months), then you won't be charged for that sheet(s).

A: If the employee worked in the same agency previously since retiring and they filled out the new form then, we do not need one. Though if they are returning to a different agency a new Form 6751 would need to be sent.
