Non-Designated Furlough Selection Request Forms and Notices: These sample forms are being offered as tools to assist agencies in the administration of furloughs during the non-designated months. They are not required for use and may not be deemed appropriate and/or useful to all agencies, based on their particular circumstances.
REMINDER: When devising your agency's plan for routing of these forms, please remember that the turnaround time (approval and return to employee) must allow for the required seven (7) day notification pursuant to 101 KAR 5:015.
Request Forms and Notices:
